NHS Birthday Celebrations: Today the National Health Service marks its 75th Anniversary, founded thanks to the efforts of the Labour Party and in particular Aneurin Bevan, following on from the 1942 report by Liberal economist William Beveridge which had proposed wide-reaching social reforms to tackle the “five giants” of “Want… Disease, Ignorance, Squalor and Idleness” (Idleness meaning of course unemployment.) The pictures in this post come from the 65th anniversary ten years ago today, Friday 5th July 2013.

Beveridge was opposed to means-tested benefits which he stated were unfair on the poor, and although the wartime coalition government set up committees to look at how his reforms could be put into practice they were from the onset opposed by many Conservatives and some on the Labour right.

But Labour stood in the 1945 elections promising to put the proposals into effect, and after their victory passed the National Insurance Act 1946, the National Assistance Act 1948, and the National Health Service Act 1946 and other Acts to do so, founding the UK’s modern welfare state.

Bevan was the wizard who integrated the multifarious per-existing medical services into the single functioning state body of the National Health Service, though he had to make many compromises to do so, working against considerable opposition both political from the Tories and from the medical bodies which were dominated by old men, many with fixed ideas.

The 75 years since then have seen many changes in the NHS, not all for the good, with both Conservative and Labour governments determined both to penny-pinch and bring in private enterprise to profit in various ways from our health service. The last 13 years have been particularly tough for the NHS, with disastrous reforms brought in and cuts which have made it impossible for NHS workers to provide the services and care they desperately want to do.

Covid of course didn’t help – and the failure to provide proper protective equipment, with too many contracts going to cronies who didn’t deliver – and fraudsters – was a real disaster, killing far too many health and other key workers. Despite a magnificent effort by health workers too many died, and the only reward for the huge efforts made by the NHS was claps.

Currently we have a government which refuses to talk sensibly with workers across the health centre, instead simply saying that the below inflation rises it is offering – essentially wage cuts – are fair. It obviously hopes that by provoking strikes and getting its friends in our billionaire owned right-wing press to condemn the strikers for the cancellations, increase in waiting lists and possible deaths it will get the public on its side against the NHS.

Instead we need policies which will engage with the problems the NHS faces after years of underfunding, overwork and shortages. The recently announced workforce plan at least is a plan to do something, though it seems unlikely to prove workable. The shortage of doctors, nurses and other medical staff has been clear almost since the start of the NHS, and certainly critical for many years.

We need to both increase the number of training places and tackle some of the other causes. Private hospitals rely on publicly funded doctors and nurses – and should at least have to pay the training costs for those they employ – although personally I’d prefer to see them being brought into public ownership. As well as poaching staff they also cherry-pick offering care for simpler cases but largely passing back more critical care to the NHS.

There needs to be an end to the expensive and somewhat unreliable use of agency staff, at least by the NHS setting up its own temporary staffing organisation and also by ending payments for work at above normal rates. The NHS also needs to become a much friendlier employer in terms of flexibility of shifts etc.

And of course we need to remove the caps on training for doctors, nurses and other staff, and to provide proper bursaries to cover both fees and maintenance for these vital workers. The proposed ‘apprenticeship’ scheme for doctors seems unworkable as such, but all medical course are already in some part apprenticeships, including some on the job training. Finding the additional mentors needed for more work-based schemes seems an example of cloud-cuckoo land thinking, and the setting up of a two-class system of doctors seems highly undesirable.

Importantly too we need to drastically reduce the bureaucratic workload of all doctors and health staff at all levels. Theoretically the increased use of IT could play a role in this – although the NHS has been disastrously shafted by its IT providers in the past, promising much but delivering mainly greater complexity. So far bringing in IT has increased the amount and time spent in form-filling at the expense of treating patients, partly because the various levels of administration up to the government has seized the opportunity for the greater demands it makes possible.

I was born before the NHS and looking at it in recent years I’ve sometimes wondered if my own death will be due to its stuttering demise. I hope not. But the NHS is certainly not safe in the hands of the Tories, and little that we have so far seen about Labour’s plans give me confidence it will be safe in theirs.

The two events I photographed on the NHS’s 65th Birthday were all concerned with its problems and doubts about its future, The GMB trade union came to Parliament with 3 vintage ambulances saying the NHS Is At Risk, campaigners at Lewisham Hospital celebrated hoping and fighting to keep their busy, successful and much needed hospital open and stop it being sacrificed to meet the disastrous PFI debts of a neighbouring hospital. I’d taken my bike to ride to Lewisham, and made a leisurely ride through Deptford on my way back to Westminster.

At the Ministry of Health, then still on Whitehall, Dr Clive Peedell was about to begin his 65 mile ultra-marathon run to David Cameron’s Witney constituency where he was to ceremonially bury the NHS coffin and launch the National Health Action Party plan by doctors and health professionals to revive the NHS. Their 10-point plan included “policies to restore the duty of the Secretary of State to provide comprehensive NHS care, and return the NHS as the preferred provider of services.”

NHS – Free, For All, Forever?. Five years ago on Saturday 30th June thousands marched through London to celebrate 70 years of the National Health Service, which began on 5th July 1948. In a few days we will celebrate the 75 anniversary. The pictures here are from that march.

A few days ago the Kings Fund published their report comparing the NHS to health care systems of other countries. It made depressing reading, but its findings were already well-known – and the result of government policies over the years.

Conservatives were opposed to the setting up of the NHS and voted against it on numerous occasions when Aneurin Bevan was campaigning for its setting up and taking the bill through parliament. Bevan established the three basic ideas, that it should benefit everyone, that healthcare would be free and that care would be provided on the basis of need rather than ability to pay.

Many doctors and dentists were opposed to the new system, which they saw as a threat to their lucrative private practices and this led to a number of compromises which have had serious consequences over the years.

Dentistry has never really been made properly a part of our health care system and now large areas of the country have few if any dentists who will take on NHS patients. Private medicine has continued and grown parasitically on the NHS – who train those working in it, while it now gets paid by the NHS to provide many of the simpler aspects of treatment while leaving the heavy lifting to our public service.

Both Conservative and Labour governments since its formation have made changes which have diluted Bevan’s principles. Contracting out of various services began under the Tories, sometimes resulting in disastrous lowering of standards. Proper cleaning services etc are essential in hospitals.

New Labour brought in Private Finance Initiative projects (they borrowed the idea from the Tories) which have led to huge ongoing debts for some NHS trusts – and closures of some vitally needed hospital departments to meet these.

Since the Coalition and then the Tories came back into power there has been a huge increase in the creeping privatisation of the NHS, with Andrew Lansley’s 2012 Health and Social Care Act forcing the NHS into a market-based model based on competitive tendering, which even his successor as Health Minister, Jeremy Hunt has described parts of as ‘frankly, completely ridiculous’. So much so that even the Tories have no had to row back a little.

The King’s Fund report makes clear that “The UK has below-average health spending per person compared to peer countries” and it “lags behind other countries in its capital investment, and has substantially fewer key physical resources than many of its peers, including CT and MRI scanners and hospital beds.”

Successive governments have prevented the training in the UK from expanding the numbers of university places for medical training, and a few years ago the Tories removed the bursaries available for nurses. Since the start of the NHS this had been a failure, and as the report states there are now “strikingly low levels of key clinical staff, including doctors and nurses” and the NHS is “heavily reliant on foreign-trained staff.” Only in recent days have we seen an effort by government to put forward plans to address the issue – and almost certainly one that is inadequate.

There is also the question of pay, brought to the fore in recent months by strikes by nurses and doctors. The government’s response has mainly been to refuse to enter into meaningful talks and mutter about the impossible demands being made.

Of course nobody expects junior doctors to get a 35% rise, but all that it would take to resolve the dispute is a much lower figure, combined with a commitment to make a small above inflation increase in a number of following years to reduce the gap that has opened up.

Save Our NHS March, London, 2017. On 4th March many thousands marched through London from Tavistock Square where the BMA have their headquarters to a rally in Parliament Square in protest against the cuts and privatisation of the NHS.

The Conservative Party were against the formation of the NHS from the start, and voted against both the Second and Third readings of the National Health Service Bill in the House of Commons in 1946.

The formation of a public state heath service had been advocated by Beatrice Webb as early as 1909, and the idea of a free, comprehensive and universal health provision had been gaining public support over the years, becoming official Labour policy under George Lansbury in 1934. By 1942 even the British Medical Association had proposed having regional councils running hospitals with consultants as paid staff, although they opposed the 1946 bill as they though doctors would lose money under the NHS.

But it was the 1942 report by William Beveridge, Social Insurance and Allied Services, that put the NHS firmly on the political agenda, with even Tory Prime Minister Winston Churchill suggesting in 1943 it would be a part of the rebuilding of Britain after the war, and both Liberals and Conservatives supporting consultations with doctors and others that resulted in a 1944 White Paper, ‘A National Health Service’.

When Labour’s Health Minister Aneurin Bevan put his National Health Service Bill to parliament in 1946, there was general agreement on the idea of a comprehensive health service, but the Tories were opposed to doctors becoming full-time employees and thus being unable to continue in hugely lucrative private practice. And the BMA shared their position, continuing their opposition after the Act was passed and eventually were able to force Bevan to amend the act and remove the introduction of a salaried service for GPs.

So, although it was a great day when the NHS came into being on 5th July 1948, and was from the start a little hamstrung by commercial interests – in this case of individual doctors. Dentistry has never been properly integrated into the system, and many find it impossible to get NHS dental treatment as relatively few dentists are prepared to work at the rates offered by the NHS. Even for those who are able to get registered as NHS patients, their are fees which for those not qualifying for exemption can be prohibitive despite being subsidised by the NHS.

It was a Labour government that proposed the idea of prescription charges in a 1949 Act, prompting the resignation of Aneurin Bevan from the Labour government, but it was a Conservative Government that introduced them in 1952. They were abolished under Labour by Harold Wilson in 1965 but he brought them back in 1968 though with significant exemptions based on age, income and medical conditions. Wales, Nothern Ireland and Scotland have now abolished charges, but in England each item now costs £9.35.

Since the 1990s many politicians, particularly Tories but also some of the leading figures in the New Labour government and on the right of the party have backed changing from the current universal system to a personal insurance-based system, and there has also been a huge increase in those taking out private insurance, with now around 4 million having this, mainly through the companies they work for.

Healthcare is now a massive industry in the UK, enabling the wealthy to avoid delays in receiving treatment in the underfunded NHS. The NHS also massively funds the private system, with some private healthcare providers receiving as much as 80% of their income for providing services to NHS patients. Some NHS hospitals also get in on the act by offering private care.

Many of the more routine services provided by the NHS have now been outsourced to private providers and over the years various changes in the way the NHS works have meant more and more has to be made available for tender by outside bodies. In some cases the level of services provided has been extremely poor – when I was in one hospital the outsourced cleaners had insufficient time to clean the floor under the beds, and accidentally putting down my hand I found a dirty dressing and a used needle from a previous occupant. Probably such outsourcing was a significant cause of various hospital-acquired infections.

But much private medicine is of a high standard – using the same surgeons and consultants others expensively trained by the NHS who work both for the NHS and private hospitals. The best private hospitals will have more equipment and more up-to-date equipment than the NHS hospitals that have suffered from years of cuts, and will also provide better conditions for their patients, with private rooms and more.

Having a two-tier system which is rapidly growing means that politicians, themselves wealthy and overwhelmingly representing the interests of the better off, and the more vocal wealthier groups in society, as well of course as the well-paid lobbyists for private health, are less likely to provide the NHS with the funding it needs to provide modern healthcare.

Many believe that our NHS is under threat, slowly and step by step being sold off to private healthcare companies, many based in the US, and that before long the NHS will be a name only, a branding used by private companies (as in many areas it already is.) The country is slowly being prepared for a complete move to an insurance based system, which will be brought in by politicians – Labour or Conservative- still chanting the mantra ‘The NHS is safe in our hands‘.

Housing: Focus E15 – Newham Show 2016

Housing remains a major problem, with many people and families in London still having to live in inadequate and often dangerous conditions. It isn’t that there is a shortage of homes, as many lie empty. There is a shortage, but it is of homes that people can afford to live in.

What London desperately needs is more low-cost housing. Council housing used to provide that, and by the late 1970s almost a third of UK households lived in social housing provided on a non-profit basis. Post-war building programmes, begun under Aneurin Bevan, the Labour government’s Minister for Health and Housing led to the building of estates both in major cities and the new towns that provided high quality housing at low rents, attempting to provide homes for both the working classes and a wider community.

Successive Conservative governments narrowed the scope of public housing provision towards only providing housing for those on low income, particularly those cleared from the city slums, reducing the quality of provision and also encouraging the building of more high-rise blocks, something also favoured by new construction methods of system building. Housing became a political battle of numbers, never mind the quality.

It was of course the “right to buy” brought in under Mrs Thatcher that was a real death blow for social housing. As well as losing many of their better properties, councils were prevented from investing the cash received from the sales in new housing – and the treasury took a cut too. Tenants seemed to do well out of it, getting homes at between half and two thirds of the market price, but often having bought their homes found the costs involved were more than they could afford, particularly when repairs were needed. Many of those homes were later sold and became “buy to let” houses.

Around ten years ago I passed an uncomfortable 25 minutes of a rail journey into London when a young student with a loud public-school voice explained to two friends what a splendid scheme “buy to let” was. He was already a landlord and profiting from it. You didn’t he told them actually have to have any money, as you could borrow it against the surety of your existing property – or a guarantee from Daddy – at a reasonable rate. You then bought a house and let it out, through an agency to avoid any hassle of actually dealing with tenants. Even allowing for the agent’s fees the rents you charged would give you a return on your investment roughly twice you were paying on your loan. It was money for nothing. And so it was for those who could get banks and others to lend them money, though recent changes have made it a little less profitable.

New Labour did nothing to improve the situation, and even made things worse through encouraging local councils to carry out regeneration schemes, demolishing council estates and replacing them with a large percentage of private properties, some largely unaffordable “affordable” properties and usually a token amount of actual social housing. The situation has been was still worsened since then, both by extending the right to buy to Housing Association properties and also by changes in tenure for those still in social housing. Successive governments have also driven up both house prices and rents by various policies, particularly the subsidies for landlords provided by Housing Benefit.

I’ve written before about Focus E15, a small group based in Newham whose activities have prompted some national debate. Begun to fight council moves to close their hostel for single mothers and disperse them to privately rented accommodation across the country (like Katie in ‘I, Daniel Blake who gets sent to Newcastle), having succeeded in their fight to stay in London they widened their scope to help others fight for decent housing – particularly in Newham, where the Labour council under Mayor Robin Wales was failing to deal with some of the worst housing problems in the country – while keeping large numbers of council properties empty.

Eventually Newham got rid of Robin Wales (and their campaign almost certainly helped) but the housing problems remain. A few days ago Focus E15 tweeted

One of Robin Wales’ big PR operations was the annual ‘Mayor’s Newham Show’ held in Central Park. Focus E15 were stopped from handing out leaflets inside or outside the show and on 1oth July 2016 set up a stall and protest on the main road a few hundred yards away. After handing out leaflets to people walking to the show for an hour or so, they briefly occupied the balconies of the empty former Police Station opposite Newham Town Hall on the road leading to the show ground in a protest against the Mayor’s housing record and policies.
